Australia offers its elderly what is called a Seniors Card, but only certain seniors qualify. Your eligibility will depend on when you were born.

For example, in 2017 the qualifying age will be slowly raised to 67 from 65. Some seniors may also be eligible for a Commonwealth Senior Health Card, or CSHC.

These cards give Australian seniors certain discounts on prescription meds. The card also makes seniors eligible for government assistance with their telephone costs.

If you are considered a self-fund retiree, you will likely qualify for the CSHC. The widows of war veterans who don't already have a Pensioner Concession Card may also qualify.

Information on Senior Health Partners

Senior Health Partners (SHP) is a program organized by Mount Sinai Hospital, The Jewish Home and Hospital, and Metropolitan New York Coordinating Council on Jewish Poverty.

SHP was created to help frail seniors stay in their homes even if they require nursing home type care. SHP operates on funds from Medicaid and Medicare.

The program provides social, nutritional, medical, and rehabilitation services in seniors' homes and at a central care clinic. The clinic has an excellent day program as well as a treatment and diagnostic center.

The geriatrics department from Mount Sinai hospital also collaborates in the program. The team that works for SHP is a multi-disciplinary team, consisting of a variety of medical and other professionals.
